Project Reports
Grant recipients are expected to make annual reports on their projects until the funds are spent in full. Please check your Grant Agreement for any special requirements.
Note: Project reports must include an accounting of expenses and any additional revenue sources. Documents must be submitted as Word (97-2003 .doc), or .pdf format.
| 1. Download the Project Report Form. |
| 2. Complete the form, saving it in your own files. |
| 3. Sign in, using the username and password you used in submitting the project application. |
| 4. On the "Organization Log-In" page, look for the link to Upload Report and follow the instructions from there. |
